If you are a new patient at Canyon Surgical Clinic, we will mail a new patient packet to your home. In it, you will find information about our offices and driving directions to the office. Please complete these forms, then mail (984 South Medical Drive, Suite 2, Brigham City, Utah 84302 , fax (435)723-0429 or bring them with you to your appointment.

Office Visits

Each time you visit our offices, please be sure to bring:

Current insurance cards.
A list of all medications you currently take. Include medication name, dosage, and name of the prescribing physician.
Co-payment as required by insurance.
